Clearly at first it is about aliens. I then goes on to describe how the worries of an everyday life can sweep you up but looking from a removed perspective our troubles are very petty.

I thought it could describe a child being taken to an asylum and given a lobotomy. All from the child's point of view, so the rest of the song comes through a lobotomized filter. 'It's all in my mind'

It's about suicide, EMT's, transfusions, surgeons, angels, and afterwards, in that order. And we're glad that 'the song maker' decided 'it ain't so bad' after all, and stayed with us. Cemeteries suck.
